Thank you for choosing Eagle Creek Dermatology. We are looking forward to helping you with all of your skin care needs. Here you will find everything you need to prepare you for your visit.
Before Scheduling Your Appointment
Please verify with your insurance company that our doctors are in-network with your specific insurance plan. You can do so by calling the number on the back of your insurance card or logging on to your insurance company's website.
New Patient Paperwork
When scheduling an appointment, we will need to know the name of your insurance policy, the reason for your visit, and the doctor you would like to see.
Please download, print, and fill out the forms below to bring to your appointment or you can upload and submit these forms below.
The Day of Your Visit
In order to make your experience as pleasant consider the following:
- Please arrive at least 10 minutes prior to your visit.
- Please bring your insurance card(s) with you to your visit.
- Please bring a completed New Patient Demographic Information Sheet with you to your visit.
- Our physicians encourage their patients to bring a list of medications, surgeries, and health history to your visit so we may incorporate the information into your personal health record.
- If your insurance plan requires a co-pay, that will be due at the time of visit. We accept cash, check, Visa, or MasterCard.
- Services not covered under insurance or self-pay require full payment at the time of service.
